How much are your services?

$175 for initial intake session and $150 per individual session.

Limited sliding scale options are available to those in need. Please contact Trevor to discuss these options.


What insurances do you accept?

Trevor is paneled with Aetna, EMI, HMHI (University of Utah), Select Health and United (UMR).

Trevor is currently in the credentialing process with Tricare, PEHP, BCBS and Cigna. While these applications are pending, he is pleased to offer a reduced private-pay rate of $70 for members of these plans.


What is your cancellation policy?

To protect the time reserved for your care and allow space for other clients, Trevor does require 24-hours' notice for all cancellations.

  • Cancellations with 24+ hours notice: Free of charge.

  • Cancellations under 24 hours notice: Subject to a $60 late fee.

  • Missed sessions / No-shows: Subject to a $100 fee.

Emergency situations are always handled with grace and compassion on a case-by-case basis.


What is a ‘Good Faith Estimate’

Under the federal No Surprises Act, you have the right to receive a "Good Faith Estimate" (GFE) explaining how much your medical and mental health care will cost before you begin treatment. This protection is designed to ensure complete financial transparency for clients who do not have insurance or who choose to self-pay/pay out-of-network for their therapy sessions. Your estimate will outline the total expected cost of our sessions based on our regular scheduling agreement. You have the right to request this estimate in writing at least one business day before your first scheduled service, and you can dispute any bill that exceeds your estimate by $400 or more.
